Root cause analysis is an effective tool for improving your equipment’s reliability.

But if the information and evidence needed to perform the analysis is not available, it’s limited. So how do we make sure we preserve the evidence?


You must capture all the evidence!

While your technicians are repairing the failure, you should have a designated person performing a Failure Scene Investigation (FSI). He must capture and preserve all the evidence, interview the witnesses, and organize the information. And he must avoid tainting the evidence.

In particular, the investigator should insure:

  • No evidence or information is overlooked
  • Evidence is captured and preserved
  • Witness accounts are recorded while the incident is still fresh in their mind

Careful, thorough investigation is key to ensuring that potential physical evidence is not tainted or destroyed or potential witnesses overlooked.
